Dengue Fever NS1 test in Delhi

Dengue also known as “break-bone fever” is a vector-borne viral infection that spreads from mosquito bites. It is typical to tropical and sub-tropical climates and endemic to certain world regions. Dengue can be life-threatening in severe cases. Severe cases are usually ones infected for a second time. There is no treatment for dengue, the focus remains to cure pain symptoms. In severe dengue cases, hospitalization is often needed. Dengue can pose an emergency situation if indications of shock or bleeding develop.

Generally, in most cases, dengue does not show symptoms, but when it does the most common symptoms are headache, high fever, body aches,  nausea, and rash. Symptoms usually appear after 4-10 days of infection and last for 2-7 days.

Severe Dengue: Patients infected for the second time are at greater risk for severe dengue. Severe dengue symptoms often come after the fever has gone away. 

Symptoms may include:

  • severe abdominal pain
  • persistent vomiting
  • rapid breathing
  • bleeding gums or nose 
  • fatigue
  • restlessness
  • blood in vomit or stool
  • being very thirsty
  • pale and cold skin
  • Feeling weak

Diagnostic Tests to Confirm Dengue:

There are two basic methods to confirm dengue fever in the laboratory:

  • Detection of dengue virus(NS1 detection)
  • Detection of anti-dengue antibodies (IgM and IgG)

DETECTION OF DENGUE-VIRUS: First virus is isolated by culture and then viral nucleic acid and antigens are detected.

NS1 Test and Its Findings: NS1 is a non-structural protein secreted into the blood during dengue infection. It can be detected during the acute phase of dengue virus infection, and after day 7 this test is not recommended.

  1. A positive result confirms dengue.
  2. A negative result does not rule out infection. Patients with negative NS1 results should be further tested for the presence of IgM antibodies to determine infection.

DETECTION OF ANTI-DENGUE ANTIBODIES

Antibody detection is done after the acute phase. But antibody response to infection differs according to the host’s immune system. The first antibody to appear is IgM whereas IgG appears secondary to it.

Primary antibody response is a slow increase in IgM. IgM is detectable in 3 to 5 days in 50% of patients, day 5 in 80%, and day 10 in 99% of patients. IgM levels peak two weeks after the onset of symptoms. IgM levels decline back to undetectable levels after two to three months.

IgG is detectable at low titers at the end of the first week of illness and is still noticeable after several months and even for life.

Normal Ranges Adult: 

  • IgG 6.0 – 16.0g/L.
  • IgM 0.4 – 2.5g/L.

Other Tests Required in Dengue Fever:

Another feature common to dengue is thrombocytopenia which may lead to vascular leakage, shock, and organ impairment.  Thrombocytopenia may occur either due to decreased production of cells from the bone marrow or from increased peripheral destruction of platelets.

A high mean platelet volume(MPV) in dengue patients indicates enhanced platelet destruction. Hence MPV is another test to be performed in dengue.
